Appointments

To schedule an appointment please call our office at 360-428-2622. We understand occasional circumstances prevent you from keeping your scheduled appointment. If possible, please notify our office in advance if you are unable to make your appointment, please notify the clinic atleast 4 hours in advance of your appointment time to avoid a no show / late notice charge. Our scheduling coordinators will do their best to re-schedule you promptly.

Insurance and Billing

Please call and speak with our Billing Department for information specific to your insurance company and plan.

Payment Options

We accept checks, cash or credit cards. Please speak with a representative from our Billing Department regarding any insurance or billing questions.
