Information for patients scheduled for a COVID-19 vaccine appointment

Vaccine Information:

We will be providing the Pfizer-BioNTech COVID-19 vaccine for our patients 12-21 years of age (as it has emergency use approval for this age group).

For detailed information regarding the Pfizer vaccine, see these fact sheets:

What to expect at your vaccine appointment:

Please arrive 10 minutes prior to your scheduled appointment time.

Plan to be here for about 60 minutes.

You will check in, fill out a pre-vaccination screening and acknowledgement form and have time to ask questions before you receive the vaccine.

Following your vaccine, you will be observed for 15-30 minutes in a waiting area.

Before you leave, make sure you know when your second dose is scheduled. This is a required vaccine booster, 3 weeks following your first dose.

One Parent/Guardian is required to be present for both the initial vaccine appointment and the vaccine booster.

Currently there is no "out of pocket" expense, however your insurance will be billed for the administration of the vaccine only.
